The identification information is obtained from the sample household listing and will be given to you by your supervisor.\n\n\n\nThe following are key points in completing the identification section:\n\n\u00b7\tWrite the name of the place or locality in which you are working.\n\n\u00b7\tWrite the name of the head of the household that you are to interview.\n\n\u00b7\tRecord the Cluster number, Household number, and the code for the region in the \n\nboxes to the right of those lines.\n\n\n\nYou will fill in the rest of the cover page after you have conducted the interview. See the instructions in Section C (RETURN TO COVER PAGE) below.\n\n\n\nTo complete the Household Questionnaire, you will need to find a suitable respondent. Any adult member of the household who is capable of providing information needed to fill in the\n\nHousehold Questionnaire can serve as the respondent. If an adult is not available, do not interview a young child or a neighbour; instead, go on to the next assigned household, and call back at the first household later.\n\nGenerally you will ask a single individual in the household for the information you will need to complete the household questionnaire.
