I have received an explanation of the procedures for my answers to be kept private and confidential. I understand that my participation in this study is voluntary. I understand that information obtained in this study will be kept confidential, and that any contact information I choose to provide will only be used to notify me of potential COVID exposure or to request follow-up on the answers I provide and will be discarded six months after this data collection period is over.”
