[By answering no, you are affirming that the patient is stable and able to consent. If the patient is incapacitated and unable to consent, she is unable to participate in the PMS patient interview. By incapacitated and unable to consent, we mean those patients who have died or are suffering from severe complications that make them incapable of consenting (ie., coma).]
Interviewer instructions

If this is incorrect and the patient is NOT incapacitated, go back and select “No” at A5. If the patient is incapacitated, proceed with interviewing their provider.
If you have already submitted this patient’s interview using a different form, proceed with interviewing their provider. Please indicate in your comments that you have already interviewed the patient.
