I would like to ask you questions about your health and well being. I know some of these questions may be sensitive or difficult to anser, but please try to provide an answer to the best of your knowledge. I will ask about your overall health, including both your physical and your mental health. Some of the questions may sound similar or repetitive, but I need to ask all of the questions so we have complete understanding of your health.
