If it turns out that the biological mother was not interviewed, a reason is stated as to why she could not be interviewed. This is an open question in Cohorts 1 - 4 meaning that the FW was free to list any reason. From Cohorts 5-10 the reasons are listed such that the FW was to choose the applicable reason from a list. The question also changes from being 1.15 to 1.15a in Cohorts 5-10.
