Interviewer instructions
You should have obtained documentation (vaccination (health) cards, vaccination booklets) for eligible children at the beginning of the interview. If you have not already collected the vaccination (health) card(s), ask the respondent to look for the card(s). In some cases, the respondent may hesitate to take time to look for the card(s), thinking that you are in a hurry. Since it is critical to obtain written documentation of the immunization history for all eligible children, be patient if the respondent needs to search for the card(s).



If the respondent shows you the card for a child, select YES, SEEN. If the respondent says the child has a vaccination (health) card, but she is unable to show it to you because she has lost it, someone else has it, or it is not accessible to her during the interview, select YES, NOT SEEN for that child. If the respondent says she does not have a card for her child, select NO CARD. Each response has a different skip instruction, so be careful to follow the correct skip pattern.
