Interviewer instructions
After completing the household questionnaire, you should have prepared a questionnaire for each eligible individual by filling in the identification section on the cover page. As you begin to interview the individual, you should fill in the area labeled “Interviewer Visits.” Here, you will record your own name, keep a record of your visits, and record the final date and result code. You will also be entering this information into your Interviewer’s Assignment Sheet.
