Interviewer instructions
From the card/booklet that the woman used during the antenatal visits, obtain information about the weight, hemoglobin levels; blood pressure and upper arm circumference. Record as indicated on the card- we do not allow the woman to recall any measurements. Note that we want to collect the measurements made during the very first and last visits and the corresponding dates as well. In the event that only one set of measurement was done during the entire pregnancy, record this in the boxes for the first visit and indicate 99 for the second visit.
