Interviewer instructions
This section starts by asking if any member of the household had died in the past three years. If the response is no then we skip to the household characteristics section. If the response is yes, then we proceed with column 38 and record the full name of the individual(s) who died. For each individual recorded, we ask about the sex in column 39; whether the death was registered with the civil authority in charge of death registration in column (40); the date when the death occurred (column 41); the age at the time of death in column 41a; the circumstances surrounding the death in column (42) and (43). In Column (42), we ask if the death was as a result of an accident such as road traffic accident, accidental falls etc; violence such as lynching by mobs; a homicide which is an unintentional killing of another person or suicide which is taking one's own life. In (43) we only ask this question if the deceased was a woman aged 15-49 to establish whether the death was a maternal death.
