Interviewer instructions
To complete Q.511, check Q.506 to see if the date of the most recent vitamin A dose is recorded. If the date is recorded, ask Q.512 if the child has received another vitamin A dose since the date recorded in Q. 506. If the date of the most recent vitamin A dose is not recorded in Q506, ask Q513 if the child has ever received a Vitamin A dose.

Q. 514 is asked for the children who received a vitamin-A dose according to either an entry on the child's health card or the mother's report. In asking Q. 514, emphasize that you are interested in the six-month period before the survey. Do not select 'YES' if the child received the last dose more than six months ago.
