Digital questionnaire with following data: demographics (age, sex), education, income, ethnicity, use of alcohol, smoking status, other CVD risk factors (physical inactivity and unhealthy diet), hours absence from work, travel costs to the clinic, adherence to drugs and medication adherence percentage (for patients already on treatment); data extraction from electronic health record if not available through digital questionnaire: co-morbidities, drug use (type, number of drugs, frequency), already receiving HTN care and physical examination done by the study nurse: height, weight, waist circumference, arm circumference, hip circumference, etc. In-depth interviews and focus group discussions (FGDs) with purposively selected providers and a subset of the patients (those with the highest quartile of adherence to blood pressure or blood sugar and the lowest quartile of adherence) provided information on provider and patient experiences with the application.
